First Visit

Thank you for choosing our office for all your allergy needs. It is our goal to ensure your experience with us is second-to-none. We have created a comfortable environment for you and our friendly, professional staff is always available to assist you with any questions/concerns you may have.

 

We ask that you arrive on time for your first appointment. Please complete the necessary online forms prior to your visit. The information collected will assist us in making an accurate diagnosis. In addition, please bring a photo ID and your insurance card. We also ask that any co-pays be paid at the time of service.

Once all the necessary health history information is collected and reviewed, your physician will discuss any tests that may need to be performed to help identify the underlying problem. This may involve X-rays, blood tests, skin tests for indoor and outdoor allergies, skin tests for food allergies and/or a lung function test. If these tests are done during the initial visit, your appointment may be between one to two hours. In some instances, we may schedule a separate appointment to conduct the necessary tests.

Financial

We strive to provide each of our patients with affordable, high-quality care. We offer a variety of payment options to help meet your needs. Please ask us about financing options, credit card payments and bank drafts.

 

Our staff is always available to assist you and answer any questions you may have concerning your account. If special arrangements are needed to help accommodate your financial needs, please speak to a member of our team prior to service. We will try to work out a suitable payment arrangement to help make your treatment more affordable.

As a courtesy to our patients, we will file insurance claims for you. We will make every attempt to help you receive the maximum insurance benefits entitled to you; however, you are personally responsible for your account. If your account has not been paid within 30 days, you will be billed for the remaining balance.

Mission Statement

Dr. Haydel's mission is to give state-of-the-art medical management in his field and to facilitate timely and expert referral when necessary. He is still driven by what the Dean of his medical school told his class the very first day: "The better person you are, the better doctor you will be."

Prescription Refills

We ask that you bring all prescription medication(s) with you to your appointment, even if it was prescribed by another physician. Please request all refills of continuous medicines during your appointment.

 

In the event of your prescription expiring prior to your next appointment, contact us at (985) 857-8271. Generally, we can request for a prescription to be refilled at your pharmacy within 24 hours.
